Let’s Talk About Preventing Muscle Loss on a GLP-1

GLP-1 medications can be incredibly helpful for weight loss, especially for people who have struggled with appetite, cravings, insulin resistance or losing weight through diet and exercise alone. But there is one part of the conversation that deserves a lot more attention: losing weight is not automatically the same thing as improving body composition. When weight comes off, especially when it comes off quickly, some of that weight can come from lean muscle along with body fat. That is why preserving muscle should be part of the plan from the beginning.

One of the biggest reasons muscle loss can happen during GLP-1-assisted weight loss is simply that people are eating much less. Reduced appetite is one of the reasons these medications work so well, but if overall food intake drops dramatically, protein intake often drops right along with it. Protein is important because your body uses it to maintain and repair muscle tissue. When you are eating less, what you do eat starts to matter even more, so meals should be built around nutrient-dense foods and enough protein to support your individual needs.

That does not mean everyone taking a GLP-1 needs to obsessively count grams of protein or live on protein shakes. Your ideal intake depends on your body size, activity level, age, health history and other factors. The bigger point is that reduced appetite should not turn into nutritional neglect. Foods such as eggs, fish, chicken, Greek yogurt, cottage cheese, beans, lentils, tofu, lean meats, nuts, seeds and appropriate protein supplements can all help make it easier to maintain adequate intake while eating smaller portions.

Strength training is just as important. Muscle responds to use, so resistance exercise gives your body a reason to hold onto it while you are losing weight. This does not require spending hours in the gym or suddenly becoming a bodybuilder. Weights, resistance bands, machines, body-weight exercises and functional movement can all be effective. What matters most is consistency and choosing a level of exercise that is appropriate for your current health and fitness level.

It is also important not to let the scale become the only measure of success. A bathroom scale cannot tell you whether the weight you lost came primarily from fat or whether a significant portion came from muscle. Muscle supports strength, mobility, glucose metabolism, physical independence and overall metabolic health, which is why the better goal is not simply to lose weight as fast as possible. The goal is to improve body composition while maintaining strength, energy and function.

Another concern we see with GLP-1 medications is people eating too little because they simply are not hungry. Appetite control can be helpful, but chronic under-eating can make it difficult to get enough protein, healthy fats, vitamins, minerals and other nutrients. If you are consistently skipping meals, struggling to eat enough, feeling weak or fatigued, or losing weight much faster than expected, that is something worth discussing with your provider.
