Tired, Gaining Weight and Low Libido? Blood Tests to Ask For

If you are exhausted, gaining weight despite not dramatically changing your habits, and wondering where your sex drive disappeared to, it is easy to assume you are simply getting older. Those symptoms are common with age, but they are also common with hormonal, thyroid, metabolic, nutritional, and other health changes that deserve a closer look.

The frustrating part is that a routine annual physical may come back “normal” even though you still feel anything but normal. Basic laboratory testing is useful, but it does not always provide enough information to explain symptoms such as fatigue, unexplained weight changes, brain fog, reduced libido, poor sleep, or loss of muscle.

That is where a more thorough blood work evaluation can become valuable.

Start With the Basics, But Look at the Whole Picture

A complete blood count, or CBC, can help identify issues such as anemia that may contribute to fatigue and weakness. A comprehensive metabolic panel, or CMP, provides information about blood sugar, electrolytes, kidney function, liver function, and several other markers that give your healthcare provider a broader view of your health.

Iron deserves additional attention as well. Someone can have a CBC that appears relatively normal while iron stores are running low. Depending on your symptoms and history, your provider may consider an iron panel and ferritin testing. Vitamin B12, folate, and vitamin D may also be evaluated when fatigue, weakness, mood changes, or other symptoms suggest that nutritional status could be part of the picture.

Is Your Thyroid Really “Normal”?

Thyroid dysfunction can create a broad collection of symptoms, including fatigue, weight gain, feeling cold, constipation, dry skin, hair changes, low mood, brain fog, menstrual changes, and reduced libido.

A TSH test is commonly included in routine blood work, but your healthcare provider may determine that additional thyroid testing, such as free T4 and other thyroid markers, is appropriate based on your symptoms and medical history. The important point is not rely solely on individual numbers. Thyroid results need to be interpreted together and in the context of how you actually feel.

Hormones Can Tell Another Part of the Story

For men experiencing fatigue, reduced libido, declining strength, changes in body composition, or erectile concerns, an evaluation may include total testosterone, free testosterone, and SHBG. Testosterone levels can vary, so timing and repeat testing may matter when evaluating possible hormone deficiency. TRT may be recommended.

For women, symptoms such as weight changes, fatigue, brain fog, poor sleep, vaginal dryness, irregular periods, and reduced libido may occur during perimenopause or menopause. Depending on age, menstrual history, symptoms, and other factors, a provider may consider tests involving estradiol, FSH, LH, testosterone, SHBG, or other hormone markers. Hormone levels can fluctuate substantially during perimenopause, so laboratory results should never be interpreted in isolation. HRT may be recommended.

What About Metabolic Testing?

Weight gain around the middle, increased hunger, energy crashes, and difficulty losing weight may also have a metabolic component.Testing may include fasting glucose, hemoglobin A1C, cholesterol and triglycerides, and additional metabolic markers when clinically appropriate. These results can help provide a clearer picture of blood sugar regulation, cardiovascular risk, and overall metabolic health.

Your Lab Results Are Only Part of the Evaluation

There is no single blood test that explains fatigue, weight gain, and low libido. Those symptoms can overlap across thyroid dysfunction, hormonal changes, anemia, nutritional deficiencies, insulin resistance, medication effects, poor sleep, chronic stress, and numerous other conditions.

Looking for one abnormal number rarely works.
